Appointment Guidelines

In order to assure that your visit and/or experiences optimal, we recommend that you familiarize yourself with the following guidelines as well as suggestions.

Preparing for Your Appointment

It is essential that you bring your insurance card to your visit, as well as a photo ID and any other referral forms that might be relevant to your visit.

Insurance provider policies state that you cannot be seen by a physician without presenting a valid Insurance ID card.

At Your Appointment

We ask that all New Patients arrive 30 minutes prior to their appointment time and established patients should arrive 15 minutes prior, in order to allow our staff to make copies of your cards and collect co-payments.
